Beating Heart Surgery - a hole new concept [Performed first time in eastern India] : -
When 44-year-old Himanshu Saha, suffered myocardial infarction, better known as 'heart attack',it brought about an rare and serious complication - Post infarction VSD.
Described as the death of a portion of heart muscle due to lack of blood flow owing to critical blockage in one or more coronary arteries, myocardial infarction the pumping capacity of the heart proportionately to the extent of damage. Mr. Saha's case was complicated by the fact that the infarction or muscle death was on the posterior portion of the heart which progressed to create a hole in the wall separating the right and left ventricles. While a hole in the heart is the commonest heart problem in children, a VSD acquired in adulthood is equally rare.
An acquired VSD causes major circulatory alteration and calls for emergency intervention. Normally major intracardiac repairs of this kind are performed on total cardiopulmonary bypass when the patient is put on heart lung machine, which takes care of maintaining blood circulation and breathing, the heart is stopped and operated upon. But the use of heart lung machine itself increases the possibility of post-operative complications like stroke or blood clotting.
To do away these problems, a team of Doctors, led by Dr Satyajit Bose, Consultant Cardiothoracic Surgeon, Apollo Gleneagles Hospitals performed the repair surgery on beating heart. "Although coronary bypass is carried out on beating hearts in many parts of the world and is the standard norm in our hospital, an intracardiac repair without heart lung machine is a feat tried for the first time in the world", said Dr. Bose.
Explaining the operation, Dr. Bose said, "We used a Transesophageal Echocardiogram (TEE), during the surgery to visualize the posterior side, which gave a very clear picture of the location and size of the hole from outside. Thereafter we stabilized a small portion of the heart and stitched the hole."
Once the problem caused by the sudden abrupt change in circulatory dynamics by the VSD, was addressed and solved, the bypass grafts could be done in normal course.
With this rare surgery performed successfully, Collate once again proved its competence to deliver innovative healthcare of international standard.

Thus it became obvious that he needed a curative therapy. Dr Aftab Khan, In-Charge, Department of Electrophysiology of the same hospital performed an Electro physiologic (EP) Study, followed by Radio Frequency Ablation (RFA) where aberrant pathways are destroyed.
'This case was particularly difficult as the abnormal pathways were so close with the normal cells, a slight faltering could necessitate pacemaker', said Dr. Khan.After the successful treatment, carried under local anesthesia, Sriparno was released the next day, with no heart medications at all.
Today he is no different from any other kid, jumping, playing and weaving dreams of being an engineer when he grows up.
